Care Coordination

Care Coordination is a comprehensive “advocate” service that collaborates with partner organizations to provide case management and benefit advocacy services for person-centered care solutions to improve quality and well being for individuals and their loved ones.

Services include:

• Benefit Advocacy Services
• Documentation Planning
• Care Planning Tools
• In-Home Case Management 
• Family Planning and communication services
• Placement services 

Case Management:

Benefit advocate that is readily available to answer questions regarding benefits eligibility. Case management services that provide individuals with assistance with navigating through the government benefits and healthcare system (SSI, SSDI, IHSS, and  Food Stamps).
